Из коробки /usr/lib/squid/pam_auth всегда возвращает ERR, т.к. нет pam-конфига для squid, а в /etc/pam.d/other прописано pam_deny.so.

Создал такой конфиг и у меня всё заработало:
# cat /etc/pam.d/squid
#%PAM-1.0
auth     include        system-auth
account  include        system-auth
session  include        system-auth
